Acceptance of Terms
By accessing or using the Salt Lake Occupational Therapy website at saltlakeot.com (the "Site"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, please do not use the Site. These Terms apply to all visitors and users of the Site.
Description of Services
Salt Lake Occupational Therapy ("SLOT") provides a website that allows prospective clients to:
- Learn about our pediatric occupational therapy services
- Request a free 20-minute consultation by submitting a booking inquiry
- Contact us with questions or inquiries
- Access educational content through our blog
Submitting a booking inquiry through this Site does not constitute a confirmed appointment or the establishment of a therapist-client relationship. All appointments are confirmed separately by SLOT staff.
Not Medical Advice
The content on this Site, including blog posts, descriptions of services, and any other materials, is provided for informational and educational purposes only. It does not constitute medical advice, diagnosis, or treatment, and should not be relied upon as such.
Always consult a qualified healthcare professional — including a licensed occupational therapist — before making decisions about your child's health or therapy. If you believe your child has a medical emergency, call 911 or your local emergency number immediately.
Booking & Consultations
Booking requests submitted through this Site are inquiries only. SLOT will confirm appointments via email. The free 20-minute consultation is offered at no charge to help families determine whether SLOT's services are appropriate for their child's needs. SLOT reserves the right to decline or reschedule any booking at its discretion.
Clinical therapy services, separate from the free consultation, are governed by separate service agreements, consent forms, and fee schedules provided at intake.

You may view and print content from this Site for personal, non-commercial use only. You may not reproduce, distribute, modify, or create derivative works without our prior written consent.
