Privacy Policy

Introduction
Salt Lake Occupational Therapy ("SLOT," "we," "us," or "our") respects your privacy and is committed to protecting the personal information you share with us. This Privacy Policy explains what information we collect, how we use it, and the choices you have regarding your information when you visit our website at saltlakeot.com (the "Site").
By using this Site, you agree to the collection and use of information in accordance with this policy. For information specifically related to protected health information (PHI) under HIPAA, please review our HIPAA Compliance Notice.
Information We Collect
We collect information you provide directly to us, including:
- Booking requests: Name, email address, phone number, preferred appointment time, and notes about your child's needs.
- Contact form submissions: Name, email address, phone number (optional), and your message.
- Communications: Any information you voluntarily provide when emailing or contacting us.
We also collect limited anonymous measurement data when you visit our Site:
- A page path without query strings and a coarse event name (such as a page view or completed form)
- The origin and path of the referring page, without query strings
- An allowlisted CTA category when a tracked CTA is used, plus coarse scroll and device bands
This measurement is stored only as daily aggregate counts. We do not store IP addresses, browser or user-agent details, cookies, persistent identifiers, form values, typed text, coordinates, or other personal information as part of this measurement.
How We Use Your Information
We use the information we collect to:
- Schedule and confirm your free 20-minute consultation
- Send you appointment confirmation and reminder emails
- Respond to your inquiries and contact form submissions
- Improve our website and services
- Comply with legal obligations
We do not sell, rent, or share your personal information with third parties for marketing purposes.
Information Sharing
We may share your information with:
- Service providers: Third-party vendors who assist us in operating the Site and providing services (email delivery, calendar scheduling). These vendors are contractually obligated to keep your information confidential and use it only for the purposes we specify.
- Legal requirements: We may disclose information if required by law or in response to valid legal process.
- Business protection: To protect the rights, property, or safety of SLOT, our clients, or others.
Cookies & Tracking
Our Site uses session cookies necessary for core functionality (such as the booking widget). We do not use tracking cookies, third-party advertising cookies, or behavioral tracking technologies. Our first-party anonymous measurement does not use cookies or persistent identifiers and honors browser Do Not Track settings. You can configure your browser to refuse cookies, though this may affect certain site features.
Anonymous measurement choice
We use a first-party, vendor-free aggregate view to understand which public pages are useful. It records only daily counts of coarse page views, CTA visibility or clicks, scroll-depth bands, navigation categories, device bands, and booking or contact outcomes. It never records form values, typed text, child or family information, coordinates, session replay, or persistent visitor identities.
No third-party heatmap or recording script is loaded. Your browser's Do Not Track setting is honored automatically, and you can also opt out below. Opting out does not affect booking, contact, newsletter, or reading functions.
Anonymous measurement is currently enabled unless Do Not Track is on.
Data Retention
We retain booking and contact information for as long as necessary to fulfill the purposes described in this policy and to comply with our legal obligations. Booking records are retained for a minimum of seven years in accordance with Utah healthcare record-keeping requirements.
Data Security
We implement appropriate technical and organizational safeguards to protect your personal information against unauthorized access, alteration, disclosure, or destruction. All data transmitted through this Site uses TLS/HTTPS encryption. However, no method of transmission over the internet is 100% secure, and we cannot guarantee absolute security.
Your Rights
You have the right to:
- Access the personal information we hold about you
- Request correction of inaccurate data
- Request deletion of your personal data, subject to legal retention requirements
- Opt out of any marketing communications (we send very few)
To exercise any of these rights, contact us at lauren@saltlakeot.com.
Children's Privacy
Our Site is directed to parents and guardians, not to children. We do not knowingly collect personal information directly from children under the age of 13. Information about a child provided in a booking or contact form is provided by the parent or guardian and is used solely to facilitate care.
Changes to This Policy
We may update this Privacy Policy from time to time. We will notify you of significant changes by updating the "Last updated" date at the top of this page. Continued use of the Site after changes constitutes acceptance of the revised policy.
